The Space Shuttle Challenger disaster showed how “groupthink” can lead a team to ignore or discount warnings.

Review what you have learned about effective teamwork from the video and, regarding the statements of Roger Boisjoly and other Morton Thiokol engineers, respond to the following:

  • How would you have overcome NASA’s insistence on launching?
  • What approach would you have used to facilitate deliberative decision making and avoid groupthink in this case?
