      example, the cardiothoracic surgical department would give a presentation

      a result of treatment (open heart surgery) or suffered any type of
      infection or complication (myocardial infarction). Ways to identify
      potential problems, protocols to correct problems once identified, and
      plans to reduce mortality and morbidity problems in the future (improved
      antiseptic techniques, use of more advanced surgical techniques and
      machinery, better pre- and post-surgical nursing care, improved patient
      screening techniques, etc.), are discussed and implemented. This
      conference is an example of a PSO that has existed in hospitals for many
      years and is used to improve patient safety, is a real-time teaching
      tool, and is actually mandated by governmental and accrediting agencies.
